Where stroke patients are taken in Moscow
With a stroke the hospital is chosen neither by the patient nor by the crew but by the routing system: the patient goes to the nearest regional stroke centre with round-the-clock CT, thrombolysis and neurosurgery. Moscow has more than twenty; the first 4.5 hours decide whether the person will speak and walk again.
What to do right now
- The 4.5-hour window
Thrombolysis — dissolving the clot — is possible only within 4.5 hours of symptom onset; mechanical clot removal up to 6–24 hours in selected cases. Hence the rule: do not wait and do not choose.

Sklifosovsky, City Hospital 1 (Pirogov), City Hospitals 15, 31, 52, 67, Botkin, Yudin, Inozemtsev, 13, 29, 36, 68 and others, distributed across the city.
- How the hospital is decided
The admissions coordination centre issues an order to the nearest centre for the call address, taking load into account — not necessarily the one in your district.
- What happens at admission
A head CT within the first 20 minutes, a decision on thrombolysis, then the stroke intensive care unit. Relatives will be asked to wait — that is normal.
- Rehabilitation and transfer
After the acute period, two to three weeks, the patient can be transferred to a rehabilitation centre or a private clinic; we move patients with medical escort.
Call an ambulance if
- Drooping face, weakness in an arm or leg, disturbed speech — call at once
- Sudden, unprecedented headache — possible haemorrhage
- Sudden vertigo with loss of coordination, double vision, loss of part of the visual field
- Symptoms that resolved within minutes — a transient attack also requires admission: one in three is followed by a stroke within days
What not to do
- Do not give blood-pressure pills "to lower it before the ambulance" — a sharp drop is dangerous in stroke
- Do not drive the patient yourself if a crew is available: the crew warns the stroke centre in advance
- Do not ask for "the best hospital" — the nearest centre with a CT scanner matters more
- Do not give food or drink — swallowing is often impaired
What the crew does
- Stroke scale assessment, time of onset, blood sugar, ECG, blood pressure
- Dispatch order to the nearest stroke centre and advance notification of the admissions team
- Transport with monitoring, oxygen and correct head positioning
- Handover to the neurologist with the exact time of onset, which determines treatment

Can we choose the stroke centre?
Not in an emergency — only the nearest. Privately, after stabilisation, for rehabilitation or continued treatment; we transport to the clinic of your choice with escort.
How much does admission with a stroke by a private crew cost in Moscow?
Call-out 6,000 ₽ for a standard crew or 15,000 ₽ for an ICU crew, transport 6,000 ₽ per each started 30 minutes inside the MKAD; evacuation by dispatch order from 15,000 ₽ by level. Treatment in the centre under insurance is free.
What if the stroke happens at a country house 100 km away?
Call 103 locally — the nearest regional stroke centre. Transfer to Moscow comes after stabilisation, by ICU ambulance; we do this regularly.
